What is congestive heart failure?


The simple view is that it is the inability of the heart to keep up with the need to circulate the blood through the peripheryHeart ECG reading and to the center of all the organs as needed by the organism as a whole. Causes can be found in both the heart and the rest of the organism influencing the heart.

 

The heart itself can be weakened by lack of oxygen, such as in hardening of the arteries; a previous heart attack; alcohol abuse causing the so called cardiomyopathy; or various nutritional deficiencies. From the outside, toxins, viruses, and thyroid abnormalities can also damage the heart muscle. Most importantly, lately, anti-inflammatory medications (NSAIDS) like Vioxx and others have been shown to lead to either frank congestive heart failure, or cause fluid retention and hypertension which may make the situation worse.

 

There is, however, more to this problem then “meets the heart”. New paradigms are emerging recognizing that this is a “whole body problem” rather than just an isolated cardiac issue. (We will discuss this more in depth later.)

 

Symptoms of congestive heart failure

The most common signs are shortness of breath, swelling of the legs, accumulation of fluid in the lungs, tiredness, and an inability to keep up with work or exercise demands because of rapid loss of strength and more.

Is this a “Hidden” condition?

Romantic SeniorsIt is estimated that several million people in the US have CHF. The more shocking news, however, is that probably several million more have a very early stage of this condition which, while it would not be classified as disease yet by the medical profession, nevertheless causes a functional impairment  for the afflicted person.

 

At this early stage the person might seek help for a “mere” fatigue, slight shortness of breath when going up the stars, etc. Generally, a doctor will perform a blood test and/or a cardiac ultra- sound (checking the function of the left ventricular) if the patient presents with concerning symptoms, such as shortness of breath.

 

Unfortunately these tests are very frequently “normal” even in advanced stages, let alone in the beginning. The diagnoses may be missed completely. No treatment is then generally offered at this time although clearly an intervention might slow down the progression of the illness.  

 

The medical treatment of congestive heart failure

 

When disease is clearly present the accepted treatment modalities consist principally in medications such as ACE inhibitors (medications used also, for example, in the treatment of high blood pressure, but used here for the purpose of preventing changes in the heart muscle), diuretics and digoxin. A salt restrictive diet and treatment of other contributing illnesses is recommended.  

 

Natural and nutritional approaches

 

Several natural herbs and amino acids are known to have a positive influence on the heart strength, although most of those approaches are not backed up by rigorous clinical studies.

The pre-eminent role of Berberine in CHF


The plant alkaloid berberine is an exception to this rule.  It has been used in Asia for quite some time to lower blood pressure, strengthen the heart muscles, and for its ability to fight arrhythmias (rhythm disturbances of the heart beat). (1)  ItBerberine Molecular Structure has been shown to lower the frequency and complexity of premature ventricular beats and to increase the left ventricular ejection fraction (LVEF) in patients with congestive heart failure. (2-4)  

 

In a more recent study the efficacy and safety of berberine for congestive heart failure was assessed. 150 patients with CHF and with >90 ventricular premature beats in a 24 hour Holter monitor and/or with a rapid heart rate were divided into two groups. All patients were given conventional therapy for CHF consisting of ACE inhibitors, digoxin, nitrates and diuretics. Patients in the treatment group were also given berberine 1.2 to 2.0 grams/ day. The other group was given a placebo.

 

General symptoms, a 6 minute walk test, a left ventricle function test (LVEF), frequency and complexity of premature ventricular beats and quality of life were assessed after 8 weeks of treatment and during a mean 2 year follow up.

 

After treatment with berberine there was a significantly greater increase in LVEF, exercise capacity, improvement in shortness of breath-fatigue index, and a decrease in frequency and complexity of premature ventricular contractions (PVC’s) compared with the control group.

There was a significant decrease in mortality of the berberine group. Berberine was found not to cause any irregular heart beats, arrhythmias, and there were no apparent side effects. (4)
